Form Builder Functions - Basics

Clinic-Assist provides you with a number of different options for creating forms for use with both appointments and the client portal. For example, you can build a screening form that includes Q&A with logic and anatomical images, or, pre placement forms with multiple-choice selections for electronically sending and completing online.   The dynamic form builder is located under ‘Settings’ and ‘Assessments’

Creating a new form

Start by selecting the ‘Add a new assessment button’ from the top left of the page.

Select the form type as ‘Appointment Module’ and enter a relevant name.  If the form was destined for use on the 'Portal', you would need to select the type as 'Portal Form'.  For more details on ‘Client Portal Forms’ please review Form Builder – Portal Forms.

Ensure ‘Is Active’ is enabled for immediate availability of the form. 

In the right column of the initial form detail, ‘Associated Products’ will be displayed if you have already configured them. This column allows you to quickly select the products to which the new form should be associated and made available for completing during an appointment.  In this example, the Drs Consult – 30mins has been selected.

Form structures and frameworks are built by adding (and removing) rows to define new sections. Each row can then be further broken down (as Cells) and assigned functionality such as radio button control, multiple choice, standard name or text entry fields and inserting an image to name but a few. These different control elements allow a form to be built with varying levels of complexity.

Begin by selecting the ‘Add a new row’ button to present a new row across the page.

Creating row/cell content

In a new row, select the Pencil button (or double click the row) to open the cell control features. The first option available is ‘Heading’. Enter a heading name in the field provided followed by ‘Apply’. The heading option is a cell control feature. All control features are added to cells in much the same way as selecting to add a heading. For a complete list of cell control options, please see Cell Control Options

 

Splitting Cells

Create a new row and select the ‘Split Cell’ function to create more cells within the row. You can divide and subdivide according to your form requirements and complexity.

 

Removing a cell

You can remove an unwanted cell by using the ‘Trash’ button.  If you want to remove an entire row, you first need to remove all cells within the row followed by selecting the ‘Trash’ button.

Clearing cell contents

Select the required cell and use the ‘Cross’ button. This keeps the cell in place but removes only the content and content control.

Adjusting the cell size

An unbroken row will display across the entire width of the page. Dividing the row into cells provides an option for each cell width to be adjusted. The height is automatically adjusted according to its content. The width can be adjusted by first selecting the cell and then the arrow(s) that appears at the side of the cell.

Previewing your form

As you build your form you can use the ‘Preview’ button at the bottom left of the page to regularly review the basic layout and alignment.
